Plan your visit

To make your appointment as efficient as possible, please follow these guidelines:

• Plan to arrive 15 minutes before your scheduled appointment to allow time for traffic and paperwork, and to prevent long waits.
• Bring your insurance card, government-issued photo ID, and a list of any medications you are currently taking.
• Masks are optional. You are welcome to wear a mask, and if you ask the staff and providers who work with you will wear a mask as well.
• Wear loose, comfortable clothing that allows access to your injury or area in pain.
• Bring any splint, brace, or other types of medical device you may have for your injury.
• Due to severe allergies of patients and staff, please do not wear scented perfumes, colognes, or lotions to your appointment.

• Clinic is in suite C/D.
• Therapy is in suite B.

Directions and parking

Landmarks

Office is the last building on the left across from the Horne Funeral Home. You will turn into the lower parking lot. To reach the upper parking lot, go around the right side of the building and up the hill.

Parking

• Free parking lot available.
• For clinic, park in the upper parking lot.
• For therapy, park in the lower parking lot.

Finding your way

• Clinic is in suite C/D off the upper parking lot.
• Therapy is in suite B off the lower parking lot.
