Forgot MyChart Login Information
Follow these instructions if you forgot your MyChart username or password.
Reset MyChart Password
If you know your MyChart username but don’t know your MyChart password, please follow these instructions.
Step 1
On mychart.orthovirginia.com, select “Forgot login information”.
Step 2
Select “I forgot my password”.
Step 3
Enter your MyChart username, ZIP code, and date of birth, then select Next.
Step 4
Enter the two-factor authentication code.
Step 5
Create and confirm new password.
Find MyChart Username
If you’re not sure of your MyChart username, please follow these instructions.
Step 1
On mychart.orthovirginia.com, select “Forgot login information”.
Step 2
Select “I forgot my username”.
Step 3
Enter your first name, last name, zip code and date of birth, then select Submit.
Step 4
If the system can find you, you will receive an email to the email we have on file with your username. If you do not receive an email, please check your spam or junk folder.
